                        Omg badass! I had this happen my last year of medical school. 70yo lady went into a COPD exacerbation with AMS. I had my inhaler, she had ipatrop but couldn’t take it. So I turned on the oxygen to full blast and just sprayed the shit out of both inhalers in the mask to act like a neb. Not sure how well the medicine worked but she eventually became more alert and was stable until we landed. I was so slow to push that button at first when the call was made.

                                            Hey Doctor Mike, this might be a stupid question but I’m wondering if administering the epinephrine when it tenses up the body puts you at any risk for blood clots when you’re at a high elevation. I know that they always talk about moving the body around and being careful about sitting for too long on planes and I’m curious if there’s any risks involved. Good job for saving that mans life! You’re truly an incredible doctor! I always get so anxious with anything medically related and around doctors but I’d trust you with my life!👍🏼

                                                              I'm a lifeguard. It may sound like the classy summer job where you walk around in a swimsuit and get a tan and do nothing, and sometimes it is. But we are trained as professional rescuers. The only people with higher certifications than us are EMTs and medical doctors. We are the real first responders in an emergency situation.

                                                              And we are not allowed to have epipens in our first-aid kits.

                                                              Because we are not trained in any sort of medications other than glucose tablets and gatorade for diabetics (thanks, Red Cross), the legal ramifications if we were to make a mistake are too astronomical, and our facilities are not legally allowed to carry medication of any sort. Not even Tylenol. If a patient has an EpiPen, we are not allowed to handle it. They can administer it themselves, and with permission we can kind of hold their hand steady if it's shaking (because they're literally DYING), but if the person has fallen unconscious due to anaphylaxis by the time we get there, we are not allowed to do a thing except keep their heart pumping.

                                                              Without oxygen, the brain starts to die in 7 minutes. EMS takes on average 3-5 minutes to get to my location. Assume 30 seconds to a minute to make the 911 call and speak to dispatch. We have a minute to a minute and a half to recognize the problem and make the 911 call.

                                                              We have the same problems with stroke victims. Because we cannot administer any NSAIDs to thin out the blood clot, having a stroke around us has a lower chance of survival than having one around your Google-friendly pal who isn't under legal obligations.

                                                              However, if Red Cross added basic medications (like epinephrine and NSAIDs) and their usages and side-effects to lifeguarding certifications, the chances of an allergy or stroke victim dying drop exponentially.

                                                                                    So I was on my Birthright trip last week as well and one of the ladies on our trip had an allergic reaction as well. She ended up getting 3 Epi Pens that day and spent about 6 hours in the hospital. EPI PENS SAVE LIVES AND SHOULD BE EVERYWHERE!!!!
